Cluster-like Headache Attributed to Pituitary Apoplexy

Abstract

|
Several studies have demonstrated cluster-like headaches attributed to secondary causes. We report a case of headache accompanying unilateral autonomic symptoms due to pituitary apoplexy. A 72-year-old man visited emergency room complaining of sudden onset right-side unilateral headache with severe periorbital pain, conjunctival injection and restlessness. Initial brain CT angiography was unremarkable. With an impression of cluster headache, he was treated
with oxygen inhalation, verapamil and sumatriptan. Later, he complained of diplopia and ptosis. Brain MR imaging
disclosed well enhancing lesion in right pituitary gland, and surgical treatment and histologic study disclosed pituitary
apoplexy. His headache relieved significantly thereafter.
